OUR REFERRAL PROCESS
1
PHONE CONVERSATION
It all starts with a call. Get in touch and we will have our initial engagement conversation to assess the request. This is an opportunity for us to understand if we can provide the service required and for you to decide if we’ll be a good fit.
2
PLACEMENT FORM AND RISK ASSESSMENT
Following the initial engagement conversation we receive a Placement Form for the local authority which we use to carry out our risk assessment. This is to understand and assess any risk the placement might incur.
3
PLACEMENT MATCH AND FUNDING ARRANGEMENT
Once the risk has been assessed and intention to place has occurred we placement match the young person to find the most suitable accommodation for them based on their unique case. A support plan is also drawn up and funding is agreed.
4
FIRST INVITATION AND INTERVIEW
The young person is invited to an introduction with their allocated key worker. An interview is also taken place to understand the unique needs of the young person and to build rapport.
5
TOUR OF ACCOMMODATION
The young person gets a tour of the accommodation and is introduced to the support staff and other young people living at the site. They are also provided with the Servco Handbook and given all their basic necessities.
6
ON-GOING SUPPORT
Once the young person starts their placement we send regular reports and updates on their support plan. We believe in constant feedback to build a good relationship and provide a stable foundation for the young person to thrive in.
